If your dog is resting peacefully and suddenly starts making a choking, gasping sound as if something is firmly stuck in their throat, pet parents often turn pale and prepare to rush to the emergency room immediately. Surprisingly, however, over 90% of these urgent-looking symptoms—where it seems like they might stop breathing at any moment—are completely natural physiological phenomena that require no emergency treatment. On the other hand, there are definitely diseases that, if mistaken for simple choking or a cold and left untreated, can lead to blocked airways and fatal consequences. They might look equally out of breath, but one is a symptom you can safely observe, while the other is a disease requiring strict management. Today, I will provide you with clear criteria to distinguish between these two situations.

You’ve probably seen your dog suddenly stretch their neck forward, expand their chest, and violently suck in air through their nose. The exact medical term for this symptom, which drives pet parents into extreme fear by looking like an asthma attack or suffocation, is “Reverse Sneezing.”
While a normal sneeze powerfully pushes foreign substances out of the nose, a reverse sneeze occurs when the “soft palate”—the area where the back of the nose meets the throat—is irritated. A spasm in the soft palate causes the dog to rapidly inhale air through the nose. As the air passes through the narrowed airway, it creates continuous harsh snapping or snorting sounds.
The causes of these soft palate spasms are diverse in everyday life. Pollen or fine dust inhaled during a walk, the scent of air fresheners or cleaning supplies in the house, temporary pressure from a collar, swallowing water or food too quickly, or even extreme excitement when a pet parent returns home can trigger reverse sneezing. Brachycephalic dogs with short snouts, such as French Bulldogs, Pugs, and Shih Tzus, experience this symptom much more frequently due to their anatomically longer soft palates.
The most important fact pet parents must keep in mind is that reverse sneezing itself does not threaten your dog’s life. The episode stops on its own within a minute or two at most, and once it’s over, your dog will return to their peaceful routine as if nothing happened. If a pet parent panics or screams, it can increase the dog’s excitement level and prolong the symptom.
When an episode occurs, gently cover your dog’s nostrils with your fingers for 1 to 2 seconds. The dog will instinctively open their mouth to breathe, and in this process, they will swallow their saliva, which naturally relieves the soft palate spasm. Alternatively, gently massaging the front of their neck is also very helpful in soothing the spasm.
If reverse sneezing is a temporary spasm, “Tracheal Collapse” is a clear respiratory disease where the airway structure itself collapses. The trachea, which acts as a pipe delivering air to the lungs, must remain robust with “C”-shaped cartilage rings. However, due to genetics or aging, this cartilage loses its elasticity and flattens out, narrowing the breathing passage. This is tracheal collapse.
The clearest difference in symptoms lies in the “sound” and the “direction of breathing.” While reverse sneezing is the sound made when inhaling air through the nose, tracheal collapse produces a distinctive metallic wheeze when exhaling as the narrowed airway vibrates. In veterinary medicine, this is described as a “Goose honking” sound. It may also be accompanied by gagging or vomiting white foam at the end of a coughing fit.
Symptoms severely worsen particularly when the leash is pulled during a walk, right after playing a vigorous game of tug-of-war with a pet parent, or during hot and humid summer weather. If tracheal collapse reaches a severe stage, poor oxygen supply leads to cyanosis, where the tongue and gums turn blue, and in severe cases, it can lead to fainting. If you are a pet parent to a small breed like a Yorkshire Terrier, Pomeranian, Chihuahua, or Toy Poodle, always observe carefully whether they make a goose-honking sound during walks.
Medical Precaution: The symptom differentiation guide explained above is for reference so pet parents can recognize the situation at home. If your dog’s cough repeats more than 2 to 3 times a day, or is accompanied by cyanosis and a reluctance to exercise, do not make assumptions on your own. Please visit a veterinary clinic immediately and consult a veterinarian for an accurate diagnosis through X-rays and fluoroscopy.
Strict management of the daily environment is essential to protect the health of small and brachycephalic dogs with vulnerable respiratory structures. The very first thing to do is “weight management.” Obesity is the most terrifying enemy of respiratory diseases. Excessive fat accumulated around the neck presses on the airway from the outside, rapidly worsening the collapse. Maintain a slim body shape where you can lightly feel their ribs through proper diet control.
Replacing walking gear is also urgent. A collar that puts direct pressure on the neck can cause fatal damage to the respiratory tract. Make sure to use a harness that wraps around the chest and shoulders to distribute the pressure. Also, you must maintain proper temperature and humidity in the indoor environment. Dry air irritates the respiratory mucous membranes and causes coughing, so it is best to adjust the indoor humidity to 50~60%.
Lastly, avoid smoking around your dog, strong perfumes, irritating air fresheners, and strenuous walks on days with high levels of fine dust or smog. For dogs with sensitive respiratory systems, these chemical and physical stimuli are major causes of inflammation and worsened coughing.
